Did you receive additional student donations after you sent in your
initial batch of school donations and order forms to the American Heart
Association? If so, simply click

additional student thank-you gifts. Be sure to mail this form in with your
additional donations in the additional reply envelope provided in your Event
Coordinator Folder. If you should need an additional reply envelope, please
